(RTTNews) - Castle Biosciences, Inc. (CSTL) announced publication of a study showing that its TissueCypher Barrett's Esophagus Test helped guide risk-aligned management decisions in patients with non-dysplastic Barrett's esophagus (NDBE).
The study, published in Foregut: The Journal of the American Foregut Society, evaluated 30 patients with NDBE who received TissueCypher testing as part of their clinical care at Keck Medical Center of USC.
Clinicians changed their management recommendations in 90% of patients after receiving TissueCypher results, including escalating management for all patients classified as intermediate or high risk and de-escalating management for 84% of those classified as low risk.
TissueCypher classified 17% of patients as intermediate or high risk for progression to high-grade dysplasia (HGD) or esophageal adenocarcinoma (EAC) within five years, while 83% were classified as low risk.
Among patients with intermediate- or high-risk results, management was escalated for all patients, including endoscopic eradication therapy for four patients and shorter-interval surveillance for one patient.
Among patients with low-risk results, management was de-escalated for 84%, including extending surveillance intervals or no longer recommending endoscopic eradication therapy.
The study also found that TissueCypher results changed management recommendations or increased physicians' confidence in their management plans for 97% of patients.
Barrett's esophagus is a precancerous condition that can progress to HGD or EAC. Patients with NDBE generally have a lower risk of progression, but traditional clinical and pathological factors may not reliably identify patients at increased risk.
TissueCypher is designed to provide a personalized five-year risk assessment for progression from Barrett's esophagus to HGD or EAC, helping physicians tailor surveillance and treatment decisions according to individual risk.
The test uses an AI-derived spatialomics approach to analyze molecular signatures in standard esophageal biopsy samples and identify patterns associated with progression risk.
Castle Biosciences said TissueCypher has been evaluated in biopsies from more than 8,000 patients and supported by more than 17 peer-reviewed publications.
The study findings were also presented on September 27 at the American College of Surgeons Clinical Congress 2026 in Washington, D.C.
